All players must bring a copy of their birth certificate and most recent report card to our office and have a photo taken for registration purposes prior to each season.
NCU players must also complete a uniform order sheet, IYSA Medical Waiver Form, the US Club Medical Waiver Form, and sign our Code of Conduct.
